On September 30, Elon Musk replied to a video of a speech calling for gender-affirming surgery to be made a crime: "Those who mutilate children should receive a death sentence." It's the second time this year he's said it. This time it reached more than 700,000 views on the platform he owns, and it was not just rhetoric. Replies underneath suggested he fund politicians to make it law. Providers of this care have already been buried in death threats. Some have shut their doors because of them.

So I want to ask you directly: how does this land for you? If you provide this care, or work beside people who do, has this week changed how you think about your safety, your practice, your staff, your patients? And what are you going to do about it, this week and this month?

Dozens of protesters with ACT UP New York and the Gender Liberation Movement picketed NewYork-Presbyterian Thursday, alleging the hospital quietly stopped accepting new minor patients and warning it not to follow NYU Langone and Mount Sinai into a paid deal with the administration. Four New Hampshire families sued HHS in federal court to protect their daughters' puberty blockers before Medicaid and CHIP funding cuts take effect October 13. At the Second Circuit, Aetna defended its categorical exclusion of facial gender-affirming surgery as "cosmetic," appealing a Connecticut injunction that found trans patients likely entitled to individualized medical-necessity reviews under Section 1557.

After 14 months offline, the 988 Lifeline's LGBTQ+ youth "Press 3" option returned without the Trevor Project, and asked whether counselors will use trans callers' names and pronouns, HHS answered only with its "gender ideology extremism" line. The Education Department formally rescinded the 2024 rule and restored the 2020 Title IX framework, stripping explicit gender identity and sexual orientation protections from federal regulations.

Nearly 100 community HIV-prevention organizations lost their federal grants as the administration redirected Ending the HIV Epidemic dollars to health departments under no requirement to pass funding through, stripping those groups of 340B eligibility on top. Senate Leadership Fund ads accusing Sherrod Brown of "forcing" childhood surgeries feature a detransitioner who isn't from Ohio and had her mastectomy at 19, in Texas.

A new Movement Advancement Project report charts anti-queer extremism's acceleration and its tight focus on trans people, while MAP's state equality maps track where those policies have landed.

International

In the UK, the equalities minister doubled down on trans exclusion from single-sex spaces at Labour's conference as advocates pressed for provisions for trans survivors of abuse. Far-right activists from the U.S. and Latin America, backed by current and former Trump officials, are building a UN coalition to counter the LGBTI Core Group and coordinate votes against sexual and reproductive rights.

Gilead's lenacapavir licensing shuts out at least 26 countries from roughly $40-a-year generics, including Mexico, Brazil, Argentina, and Peru, while MSF says it still can't buy the drug directly.

Push your medical society — The Colorado Medical Society's board voted to oppose Initiative 110, the anti-trans surgery measure on November's ballot, on the principle that medical decisions belong in the exam room, and every state society facing a similar measure can take the same stand.

Back a national ban — A conversion therapy survivor's petition asks Australia's federal government for a nationwide ban that rules out "informed consent" and "sustained effort" loopholes, since protections now depend on which state you live in.

Bring the Bill home — Lambda Legal's new LGBTQ+ Bill of Rights pairs its lawsuits with model state legislation on health, education, family, and democracy, starting in 20 states, with organizing resources rolling out through October.
